Introduction: mortality due to congenital malformations is among the main causes of infant mortality. Objective: to describe infant mortality due to congenital malformations in a region of northeastern Brazil. Methodology: cross-sectional and descriptive study, using secondary data from the Mortality Information System, on mortality in children under one year of age and due to congenital malformations, which occurred in Ceará, from 2018 to 2023. Results: among deaths in children under one year of age with congenital anomalies, the main cause was congenital malformations of the heart followed by malformations of the nervous system; they were male; had brown race/skin color; birth weight was above 2,500 g. Mothers were 25 to 29 years old; had eight to 11 years of schooling; gestational age at delivery was 37 to 41 weeks; singleton pregnancies and cesarean delivery. Conclusion: the main determinants presented in this research, which influenced infant mortality, especially due to congenital malformations, focus on improving public health policies.
